Coronavirus Update:

Beginning 3/16/2020, all counseling and psychiatric sessions will be via telehealth for the time being. Call or text us at 541-868-2004 and we will assist you.

24 Hour Notice of Cancellation Required:

Clients are required to provide 24 hour notice of cancellation to avoid being charged $100 for a missed session. Monday sessions must be cancelled by Friday at the corresponding time.

Appointment times are reserved for only one person, couple or family. 24-hour cancellation notice is required to avoid being charged for a missed session. You will be charged $100 for a missed session when 24 hour notice of cancellation is not provided. We may, at our sole discretion, make exceptions in the case of emergencies or sickness. If you need to cancel, please call or leave a message at 541-868-2004.

Our receptionist makes courtesy reminder calls the day before your session, however, you as the client are ultimately responsible for remembering to attend your scheduled appointment.

If you arrive late for an appointment, we will still conclude the session at the scheduled time. If you are 20 minutes late for an appointment the appointment will be considered cancelled and you will be billed $100 for the missed session.

There is a $50.00 fee for returned checks. 

Medical Insurance:

Our office will bill your insurance carrier as a courtesy. You are responsible to check with your insurance company regarding your coverage and to track this coverage as treatment progresses. Though our office may do so as a courtesy, it is ultimately the client’s responsibility to determine the extent of insurance coverage available. The client is responsible for the entire bill should insurance coverage or claims be denied. 

Emergencies:

Although calls to the voicemail are answered on a regular basis, we do not provide emergency services. If in crisis, please call the White Bird Crisis Line 541-687-4000, or 911 or other crisis phone lines to access emergency services as you see fit.

Clients, please sign in using the user id and password you were emailed.

1) Login to the Client Login section and you will be taken to the HOME screen where you will see a MY TO DO LIST.

2) Please click on each link under the heading MESSAGE and then read and complete each form we have initially placed in your MY TO DO LIST.

3) Once you have read the required form scroll down to the bottom of the page and enter your password and click SIGN AND COMPLETE DOCUMENT.


PASSWORD PROTECTION INFO: The information you submit is secured and no one else has access to your clinical information as it is password protected. Once logged in, you will have the option of changing your password by clicking on change password on the left top portion of the screen. In order to get back to the main page to continue with the To Do List, click the back button at the bottom of each form.


FOR CLIENTS UNDER 18: a parent or guardian will need to fill out each form for the client and electronically sign each form.
